Dear Open Records Coordinator for the "State Office of Risk Management" State of Texas agency here in Austin,

This is a Texas Open Records request in which I seek to obtain from you:

---a copy of any and all e-mail communications or other written communications referring at least once in any manner to myself or to my "Progressive Prohibitionist Religion", that at any time since January 1, 2011, were authored, typed, handwritten, sent, received, mailed, forwarded, e-mailed, or FAXed by the Chief of the Legal Services Division for your State Office of Risk Management state agency, or by any other attorney or staff member or administrator in that entire multi-purpose division of your state agency in Austin.

---a copy of any and all e-mail communications or other written communications referring at least once in any manner to myself or to my "Progressive Prohibitionist Religion", that at any time since January 1, 2011, were authored, typed, handwritten, sent, received, mailed, forwarded, e-mailed, or FAXed by the Chief of the Strategic Programs Division that oversees risk management for your state agency, or by an other staff member or administrator or official in that entire multi-purpose division of your state agency in Austin.

My legal name is John Kevin McMillan. My date of birth is April 27,1957. I was born in Lincoln, Nebraska. I am a single adult, gainfully-employed, longtime celibate, white Anglo-Saxon non-Hispanic gentleman of English, Scottish, and German ancestry.
I have resided in Austin proper here in Texas on a continuous and uninterrupted basis ever since mid-March of 1997.

Your current official online website states that Deea L. Western, J.D., is Division Chief and General Counsel for your state agency's Legal Services Division.
Deea Western reportedly joined SORM as General Counsel and Division Chief for Legal Services in April of 2015. The Legal Services Division is reportedly comprised of three departments:  Litigation Management, Special Investigations, and Compliance and Practices.
Ever since November 1, 2017, James Cox has reportedly presided over your state agency's Chief of Strategic Programs Division.  Your official website also states: "As Division Chief, Mr. Cox oversees the Risk Management, Claims Operations, Communications & Development, and Document Processing departments. He serves as the leadership for administering workers’ compensation benefits for state employees, introduces new insurance lines for the state, expands the vision for Continuity of Operations (COOP), and advances Enterprise Risk Management (ERM)."

This public-information request to your State of Texas agency was prompted in part by a much-appreciated e-mail reply letter I received this month from the California Highway Patrol state agency in Sacramento, California. That reply letter informed me in writing that on their own initiative they had chosen to refer a courtesy carbon-copy I had sent to them on legal correspondence of mine with the Austin-based Texas Secretary of State, to the Office of Risk Management Public Records Unit of the California Highway Patrol. I then provided written authorization to the California Highway Patrol state agency to process what they had identified as a public information request appropriate for their Office of Risk Management's Public Records Unit.

 ___________

I myself resided in California for one total school year during my childhood, when I attended Hillside Elementary School, a public school in Berkeley. I have not resided in California on any occasion or at any time since my second-grade year of elementary school.

I currently reside in Texas at a large gated-community apartment complex, Pebble Creek Apartments, 8805 North Plaza Drive in northeast Austin.

I have been and continue to be gainfully employed as a non-managerial staff member at one or more restaurant workplaces in the Austin area of Texas on a continuous and uninterrupted basis ever since April 27, 2002. I have been employed exclusively in the restaurant industry in Austin and nearby cities that included Round Rock and Cedar Park and possibly Pflugerville, on a continuous and uninterrupted basis throughout the entire 10-and-one-three-fourths-year period applicable to this public-information request.

When I previously worked for the State Government of Texas, one or two of my full-time respective employers, the Texas Board of Pardons and Paroles state agency along Shoal Creek Boulevard in north Austin or the Texas Department of Criminal Justice state agency in north Austin, informed me that I was being paid a "salary differential" income supplement each month because of "hazardous working conditions" at my state agency workplace. Our entire workplace in north Austin received occasional bomb threats during workshifts of mine at my state agency workplace in north Austin during that time period. We were routinely ordered by our work supervisor to leave our office building until that bomb threat was investigated and it was determined by security staff for our workplace that it was safe for us to return to work inside that building.

I lead a permanently drinking-alcohol-free (ever since 1990), permanently marijuana-free (ever since 1984), lifelong-illicit-drug-free in regard to any and all illicit drugs other than marijuana, lifelong-tobacco-free, lifelong-non-vaping, dependably civil and law-abiding and honest and vigilant, non-stalking, non-sadomasochistic and anti-S&M, non-gambling, non-pranking (I am VERY opposed to pranksterism and fraudulent communications and criminal mischief), lifelong-tattoo-less and anti-tattoos-minded, permanently jewelry-less, clean-talking (no profanity by me in my everyday conversations with others), platonicly polite lifestyle. I strongly oppose "thought-control projects" and I take pride in being privacy-respectful. I myself DO NOT generate any anonymous communications that are inflicted on anyone else in a context that violates or impinges on their own privacy rights. I oppose verbal harassment of or by anyone.

I am the founder and only approved current member of the new and non-Christian, factually-focused and public-safety-minded, non-proselytizing Progressive Prohibitionist Religion---an "Honor Society" religion with very stringent membership-eligibility requirements. That only religion of mine was officially registered by me with the Secretary of State of Texas in 1996, when I resided in Denver City, Texas. However, I later chose not to renew my registration with that state agency. I have made that decision because I am the only approved member, and because I want to wait until I enjoy fuller privacy rights and fuller access to very reliable, factually accurate news and information services in my own life for a reasonable multi-month duration of time, such as one full year, before I am willing to even consider permitting any other person to apply for possible membership in my new religion.

I am a current criminal-law complainant in APD Sex Crimes Case 2021-2440624, a new and currently active APD Sex Crimes and alleged Home Invasion Crimes case that was opened by APD on September 1, 2021. An APD-authorized DNA-swabs forensic sexual-assault-evidence medical exam was performed on me by a female forensic nurse for Austin SAFE Alliance in the early morning hours of September 2, 2021, inside the ER of Dell Seton Medical Center at The University of Texas, in central Austin. The APD Sex Crimes investigator in the current case is APD Detective William Costello. Detective Costello's office phone number is: (512) 974-4247.

I was a recent criminal-law complainant in APD Alleged Sex Crimes and Alleged Home Invasion Crimes Case 2021-0830533----an APD crime case that was activated by APD on March 24, 2021.

In late 2020, I filed an APD Sex Crimes and Home-Invasion Crimes criminal-law complaint for which APD assigned the official ID number of 20-3161213 on November 11, 2020.

One highly-regarded Ear, Nose, and Throat medical specialist whom I have consulted in Austin, Dr. David Tobey, stated to me in a recent prior year during a medical appointment I had with him that he believes that alleged home-invasion-crimes perpetrators inside my locked apartment unit at that time in northwest Austin had illegally subjected me to chloroform in my nostrils that held me unconscious during my sleeping hours as I lay alone on my own bed inside my locked apartment unit, Dr. Tobey helpfully volunteered.

The same Dr. Tobey earlier this century stated to me inside his medical clinic, then situated along Angus road in northwest Austin, that he believed that permanent damage to my own hearing capacity from continuous noise pollution in Austin that I myself had not generated and I myself was not blameworthy for, was comparable in magnitude to what an Austin Police Department officer sustains over the entire course of that APD officer's career, Dr. Tobey stated.

In the 10-and-three-fourths-year period of this public-information request, I myself have undergone at least seven separate doctor-ordered medical surgeries on myself in several respective hospitals of Austin (including St. David's North Austin Medical Center, Seton Central Medical Center in central Austin, and Heart Hospital of Austin) for potentially life-threatening medical problems I had sustained.

One medical physician whom I consulted, Dr. Carson Higgs, a first-rate family-practice physician at Austin Regional Clinic Quarry Lake location, stated to me during a medical exam I had with him that sleep deprivation could have played a role in my having sustained damage to my circulatory system for which medical surgery was ordered by doctors. Ever since mid-April 2011, I have been awakened several times per night, in the middle of my nocturnal dreams, with anal pain or an unpleasant sensation in my anus, an unpleasant nasal-inhalant sensation in my nostrils even though I do not own or use nasal inhalants, often with a ringing sensation in my eardrums, an urgent need to urinate in my apartment's bathroom, pain in my legs or hips, and wobbliness in my legs as I walked to the bathroom. Because of mysterious pain in my body I had sustained during my sleeping hours, I often had difficulty falling back asleep after I walked back to my bed from the bathroom.

My Wind River Crossing Apartments management-team-approved roommate from mid-April 2011 through the end of May 2011, Sandjaja Utama, a self-identified citizen of Indonesia and self-identified "member of the Catholic Church", stated to me several times that his "auntie" was a medical physician named "Dr. Sheila Shung" who resides with her husband in Round Rock, Texas, Mr. Utama stated. Mr. Utama stated to me that he himself was interviewed by Austin Police Department Sex Crimes Unit Detective David Silva only on the telephone during Mr. Utama's period of residence with me. That telephone interview took place after I filed a crime report with APD on April 28, 2011, for which APD's Sex Crimes Unit officially authorized a forensic DNA-swabs forensic sexual-assault-evidence medical exam on me for APD Case 11-1180447---a rape-evidence exam on me that occurred on April 28, 2011, inside the St. David's Medical Center ER in central Austin.  No criminal charges were filed by APD against anyone in that particular APD case, and Detective Silva months later stated to me that results from DNA-evidence processing of the rape kit were negative, he stated to me on the telephone.

My primary personal e-mail address, "mcmillanj@att.net", has remained the same throughout the approximately 10-and-three-fourths-year period applicable to this public-information request. My landline phone number, (512) 342-2295, has remained the same throughout the entire 10-and-three-fourths-year time period applicable to this public-information request. My cell phone number of (512) 993-7305 has been the same ever since the summer of 2019, when I became a cell phone owner and cell phone user for the first time ever.

I have resided at the following respective home addresses during the most recent 10-and-three-fourths-year time period:
---Wind River Crossing Apartments, 11411 Research Boulevard, Apt. 325, Austin, TX 78759.
---Crossland Economy Studios, Extended Stay America, 12621 Hymeadow Road, Apt. 133, Austin, TX 78729.
---Village Oaks Apartments, 10926 Jollyville Road, Bldg. 9, Apt. 902, Austin, TX 78759.
---Village Oaks Apartments, 10926 Jollyville Road, Bldg. 16, Apt. 1609, Austin, TX 78759.
---Austin Motor Inn, 11400 North IH 35 (a hotel room I rented for one week in June 2019 until I had official prior permission from the Pebble Creek Apartments management team to move into my new rental-apartment unit at the cited date and time of noon on June 21, 2019), Austin, TX 78753;

and, most recently, Pebble Creek Apartments, 8805 North Plaza Drive, Building 17, Apt. 2418, Austin, TX 78753.

Ever since and including the day I moved into my current efficiency-apartment unit at Pebble Creek Apartments on June 21, 2019, I have been and continue to be the ONLY official tenant and the ONLY apartment-management-team-approved occupant in my current bolt-locked efficiency rental-apartment unit, Apt. 2418, at Pebble Creek Apartments.

I hope to hear from you soon in response to this public-information request from myself.

Sincerely and Best Wishes,

John Kevin McMillan, public-information requestor.
